Simple & Supportive Referral Process
Our referral process is designed to be straightforward, collaborative and participant-focused.
Once your referral is received, we will connect with the participant, support coordinator or family member within 3 business days to understand their goals and support needs, so we can work with you to develop your personalised support plan.
